Highlight your community health issue by submitting content to help connect people to your issue and demonstrate its importance to the wellbeing of the community. Once you complete your submission, a member of Live Well Humboldt will review it within 5 business days and add it to the site if appropriate.*

Promising Practice submissions are managed by the Healthy Communities Institute, and you will receive information about the process and timeline upon completion of the submission form.
