Who is Live Well Humboldt?
Live Well Humboldt (LWH) is a network of community health improvement collaborators committed to:
- aligning actions to improve health across Humboldt County
- centralizing data collection and analysis to drive change
- identifying shared measures and targets for community health improvement
- engaging the community and creating broad ownership for local health
- mobilizing support to advance health equity and improve community health
Collective Impact
Collective impact is a framework that brings people together in a structured way to achieve population and systems level change, according to the Aspen Institute/ FSG Forum for Community Solutions.
Collective impact initiatives are guided by the five conditions below. LWH strives to be a collective impact initiative and continue to use this framework, together with considerations of health and racial equity, to guide next steps and CHIP implementation.
