Community Health Assessment & Improvement Plan
The CHA is a summary of data about our health in Humboldt County and the factors that influence those health outcomes. The purpose of a CHA is to help us learn about the health status of our community and identify areas for health improvement.
The CHIP is a community-wide action plan for reducing health disparities, promoting health equity and improving overall population health. The purpose of the CHIP is to ensure coordination and measurable health improvement in these priority areas throughout the county, with community partners working together toward shared goals by making the best use of resources and aligning initiatives and programs.
According to the National Association of County and City Health Officials (NACCHO), "a community health improvement process uses CHA data to identify priority issues, develop and implement strategies for action, and establish accountability to ensure measurable health improvement, which are often outlined in the form of a community health improvement plan (CHIP). A community health improvement process looks outside of the performance of an individual organization serving a specific segment of a community to the way in which the activities of many organizations contribute to community health improvement."
